This notice describes how health information about you may be used and disclosed, and how you can access this information. Please review it carefully.

Your Rights

You have the right to:

  • Receive a copy of your health records

  • Request corrections to your health records

  • Request confidential communications

  • Ask us to limit how we use or share your information

  • Receive a list of disclosures we have made

  • Receive a paper copy of this notice

  • File a complaint if you believe your privacy rights have been violated

Your Rights in Detail

Get a copy of your health records You may request an electronic or paper copy of your health records and other information we maintain about you. We will provide a copy or summary within 30 days of your request. A reasonable, cost-based fee may apply.

Request corrections to your records If you believe information in your records is incorrect or incomplete, you may request a correction. We will respond in writing within 60 days. We may deny the request but will explain why.

Request confidential communications You may ask us to contact you in a specific way (by cell phone rather than home phone, for example) or to send correspondence to a different address. We will honor all reasonable requests.

Ask us to limit what we use or share You may request that we limit how we use or share your health information. We are not always required to agree, but we will consider every request seriously. If you pay for services entirely out of pocket, you have the right to ask that we not share that information with your health insurer, and we will honor that request unless required by law to do otherwise.

Get a list of disclosures You may request an accounting of disclosures we have made of your health information for up to six years prior to your request, including who received it and why. We will provide one accounting per year at no charge; additional requests within 12 months may incur a reasonable fee.

File a complaint If you believe your privacy rights have been violated, you may contact Inspire Psychology LLC directly or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights:

You will never be penalized for filing a complaint.

How We Use and Share Your Information

For treatment: We may use your health information to provide and coordinate your care. For example, if we consult with another provider involved in your treatment, we may share relevant information with that provider.

For practice operations: We may use your health information to manage and improve our services, maintain records, and contact you when necessary.

For billing and payment: If you are seeking reimbursement from your insurance provider, we may share relevant information, such as a superbill, to facilitate payment. We will only share the minimum information necessary.

Special Protections for Mental Health Information

As a mental health provider, Inspire Psychology LLC affords heightened protections to your mental health records beyond standard HIPAA requirements. In particular:

  • Psychotherapy notes — notes recorded during therapy sessions — are held to a higher standard of confidentiality and will not be shared without your written authorization, except as required by law

  • Your mental health treatment records will not be shared without your written consent except for treatment, payment, or as otherwise required by applicable state or federal law

  • New Jersey law provides additional privacy protections for mental health records; we comply fully with both state and federal requirements

When Disclosure May Occur Without Your Authorization

There are limited circumstances under which we are required or permitted to share your information without your consent:

  • Mandatory reporting: We are required by law to report suspected child abuse, neglect, or endangerment to the appropriate authorities

  • Serious threat of harm: If we believe there is a serious and credible threat of harm to you or another identifiable person, we may disclose information necessary to prevent that harm

  • Legal proceedings: We may be required to share information in response to a valid court order or subpoena

  • Regulatory compliance: We may share information with oversight agencies to demonstrate compliance with applicable laws

  • Breach notification: We will notify you promptly if a breach occurs that may have compromised the privacy or security of your information

We will never use or share your information for marketing purposes or sell your information under any circumstances.

Our Responsibilities

Inspire Psychology LLC is required by law to:

  • Maintain the privacy and security of your protected health information

  • Notify you promptly in the event of a breach that may have compromised your information

  • Follow the duties and privacy practices described in this notice

  • Provide you with a copy of this notice upon request

We will not use or share your information in any way not described in this notice without your written authorization. You may revoke written authorization at any time.

Changes to This Notice

We reserve the right to update the terms of this notice. Changes will apply to all information we hold about you. The current version of this notice will always be available on our website and upon request.
